Golden Passport

Online applications are being accepted for a new, renewal, or replacement Golden Passport.

Through the online application, program participants can:   

• Submit a new application
• Submit a renewal application 
• Submit a replacement application
• Pay the replacement fee, when applicable
• Check the application status 

Submit your application for a new, renewal or replacement Golden Passport.

You can also print and complete the Golden Passport EASY Card application then apply in person at the Miami-Dade Transit Golden Passport Office located on the first floor of the Government Center.

When you apply, your photo will be taken. Your image will appear on your new card.

Applicants 65 years old and over must bring:

• A current valid Florida ID or driver's license with a Miami-Dade County physical address.

Applicants 64 years of age and younger must bring their:

• A current valid Florida ID or driver's license with a Miami-Dade County physical address
• A current printout from the Social Security Administration verifying eligibility
  • Proof of continued eligibility must be provided annually
• Eligible children must have a school ID or Florida ID

Your Golden Passport EASY Card is valid for 20 years if you are 65 years or older.

Social Security beneficiaries 64 years of age or under must renew their Golden Passport annually. The expiration date is printed on the card.

You can renew online, by mail, email or fax by providing copies of the required documents. Your address must be current and correct in our system.

To renew you must provide the following:

• A current valid Florida ID or driver's license with a Miami-Dade County physical address
• Your last issued Golden Passport EASY Card
• A current printout from Social Security Administration verifying eligibility

Replace your lost, stolen or damaged EASY Card at any Transit Service Center by completing the EASY Card replacement form.

To replace your lost, stolen or damaged EASY Card you must provide the following:

• A current valid Florida ID or driver's license with a Miami-Dade County physical address
• A replacement application
• Replacement Fee (checks are not accepted)

Fees:

• Free for the first replacement
• $10 for the second replacement
• $25 for each replacement thereafter

A fee waiver is available when an original police report is presented listing the card as a stolen item..
