Online applications are being accepted for a new, renewal, or replacement Patriot Passport.
Through the online application, program participants can:
- Submit a new application
- Submit a renewal application
- Submit a replacement application
- Pay the replacement fee, when applicable
- Check the application status

You can also print and complete the Patriot Passport EASY Card application then apply in person at the Miami-Dade Transit Golden Passport Office located on the first floor of the Government Center or
The City of Coral Gables Community Recreation Department, 2 Andalusia Ave, Coral Gables, FL 33134, on the second and fourth Wednesday of every month from 11 a.m. – 1 p.m. No appointment necessary. Applications must be submitted in person.
Miami Beach residents can submit their application in person at the Miami Beach Customer Service Center. Make an appointment online or call 305-673-7420.
When you apply, your photo will be taken. Your image will appear on your new card.
Disabled veterans need to bring:
- Service-connected disability letter showing percentage of disability and type of military discharge
- A current valid Florida ID or driver’s license with a Miami-Dade County physical address
Other veterans need to bring:
- Current valid Florida ID or driver’s license with a Miami-Dade County physical address
- DD214 form as proof of an honorable discharge
- Proof of income:
- Consecutive pay stubs, weekly (four pay stubs), bi-weekly (two pay stubs) if paid monthly (one pay stub) or
- Previous year's tax return
- Previous year's IRS Tax Return Transcript
- Previous year's IRS Notice of Non-Filing and Previous Year IRS Wage and Income Transcript
- Income Tax W2 wages statement
The Patriot Passport expires annually and has to be renewed. The expiration date is printed on the card.
