Patriot Passport

Online applications are being accepted for a new, renewal, or replacement Patriot Passport.

Through the online application, program participants can:   

• Submit a new application
• Submit a renewal application 
• Submit a replacement application
• Pay the replacement fee, when applicable
• Check the application status

You can also print and complete the Patriot Passport EASY Card application then apply in person at the Miami-Dade Transit Golden Passport Office located on the first floor of the Government Center or 
The City of Coral Gables Community Recreation Department, 2 Andalusia Ave, Coral Gables, FL 33134, on the second and fourth Wednesday of every month from 11 a.m. – 1 p.m. No appointment necessary. Applications must be submitted in person.

Miami Beach residents can submit their application in person at the Miami Beach Customer Service Center. Make an appointment online or call 305-673-7420.

When you apply, your photo will be taken. Your image will appear on your new card.

Disabled veterans need to bring:

• Service-connected disability letter showing percentage of disability and type of military discharge
• A current valid Florida ID or driver’s license with a Miami-Dade County physical address

Other veterans need to bring:

• Current valid Florida ID or driver’s license with a Miami-Dade County physical address
• DD214 form as proof of an honorable discharge
• Proof of income:
  • Consecutive pay stubs, weekly (four pay stubs), bi-weekly (two pay stubs) if paid monthly (one pay stub) or
  • Previous year's tax return
  • Previous year's IRS Tax Return Transcript
  • Previous year's IRS Notice of Non-Filing and Previous Year IRS Wage and Income Transcript
  • Income Tax W2 wages statement

The Patriot Passport expires annually and has to be renewed. The expiration date is printed on the card.

Who is Eligible?

1. Service-connected disabled veterans who are permanent residents of Miami-Dade.
2. Veterans, based on their military discharge, who are permanent residents of Miami-Dade and whose annual income is $36,614 or less.

Type of Military Discharge:

• Honorable Discharge - Yes
• General Discharge (under honorable conditions) - Yes
• Other than Honorable Discharge - Yes
• Inability to Adjust - Yes
• Voluntary Discharge - Yes
• Dishonorable/Bad Conduct Discharge - No

The Patriot Passport expires annually and has to be renewed. The expiration date is printed on the card.

You can renew by mail, email or fax by providing copies of the required documents.

To renew in person, disabled veterans need to bring:

1. Service-connected disability letter showing percentage of disability and type of military discharge
2. A current valid Florida ID or driver’s license with a Miami-Dade County physical address

Other veterans need to bring:

• Proof of income:
  1. Consecutive pay stubs, weekly (four pay stubs), bi-weekly (two pay stubs) if paid monthly (one pay stub) or
  2. Current income tax return or IRS Tax Return Transcript or
  3. Income Tax W2 wages statement
• A current valid Florida ID or driver’s license with a Miami-Dade County physical address
• Your last issued Patriot Passport EASY Card

Replace your lost, stolen or damaged EASY Card at any Transit Service Center by completing the EASY Card replacement form.

Fees:

• Free for the first replacement
• $10 for the second replacement
• $25 for each replacement thereafter

A fee waiver is available when an original police report is presented listing the card as a stolen item.
