Dependent Documentation

If you are adding a new dependent on your medical plan regardless of last name, you must provide proof of eligibility. The documentation must be received by February 28, 2020. Failure to provide this documentation will result in the cancellation of your dependent’s coverage, as if never covered.

Away From Home

Dependents enrolled in the Away From Home program are not affected by the network changes. Eligible dependents enrolled in the Away From Home program will still have access to the PHCS network providers for out of area services while enrolled in the program.
